Mercy Homecare's clinicians save 15 minutes during admission visits by having patients sign a single form that acknowledges several other forms were received and explained.
Before using the form, patients had to sign each form, meaning more than 10 signatures, says Ronda Howerter, director of the 110-patient agency in Independence, Kan.
When patients sign the streamlined "Admission Information Confirmation Sheet," they acknowledge that the nurse explained several documents, including the explanation of homebound status, patient rights and responsibilities, privacy statements and emergency instructions.
